this one happened last Nov., just down the interstate here http://www.youtube.com/watch?v=iCsyJas_YRE&feature=fvsr

supposedly cleared the scoreboard by 60'. apparently "conflicting traffic" and "task saturation" caused the pilot to descend a bit much.
