The way it was reported by the St Pete Times back then. This is something many may not be aware of. Two USAF F4C's and a B-57 Canberra mistakenly attack a USCG Cutter in VN with tragic results.

The Coast Guard wasn't the only friendly force fired up by the US.

Statistically speaking....considering the amount of aircraft that the U.S. had there, there were likely a few other friendly fire mishaps involving aircraft.
