I was reading somewhere online that this wasn't the first time that there had been an AD with this weapon/holster combo, but first in flight. Apparently, it is possible to cause the trigger lock to actuate and close while in front of the trigger rather than behind it as designed. With it closed in front, it is possible for it to cause an AD when the weapon is fully seated in the holster.
