When one pranged up a plane in largely avoidable incidents, taxi incidents, running out of fuel, etc etc. were they grounded? Given a new plane and told to get back out there? Was there accident investigations after incidents? for example, pilot lands gear up in one of those "he forgot to put the gear down" examples you read so much about. Were there reprecussions for those types of things?

Depends on who the pilot was, and who his CO was. I know that wheels-up landings could be 'punished' with a crate of beer or temporary desk duty. Some incidents which didn't even result in and accident often resulted in a Court Martial (for post-war low flying for example).
