Whose notebook?

A slightly different JDK quiz here, although Mike still can't play.

This is the first page from a record-setting flyer's notes from a significant flight. I've blurred a couple of dead give-aways at the end (a name and a location, and something else) but there's enough other clues in the notes I think. In fairness it's also not a warbird story, but historic aviation.

Who (the correct crew-member, please) what, where and when?

Well James, I'm guessing the note book would belong to Jim Warner, Radio Operator on the Southern Cross, recording his departure from San Francisco on the morning of 31st May 1928, on the first leg of the first successful crossing by air of the Pacific Ocean.

Does help if one is reading Sir Charles Kingsford-Smith's Biography at this point in time!!!!

Ok, fresh underwear on, drove past said Southern Cross this morning on the way to work to play 777's.
My guess this time is Charles Ulm.
