Anyone have a copy of a blank aircraft form 1 or a copy of one filled out? Love to see what they looked like and what info was documented.

Form 1 and Form 1A from the Pilot's Information File. If you are interested in WWII aircraft operations, I highly recommend picking up an original one of these books- it's a very compact bunch of information about how pilot's flew in the AAF in WWII. The Form 1A was more important than the Form 1, and the crew chief I talked to said pilot's who payed attention to Form 1A's got more attention from him than the ones who meticulously filled out their Form 1's. I threw in a copy of the Form 5 to show people how a pilot's flight information was logged in WWII. These are big files to store on my site, so I will take them down after a while. Feel free to copy them.

I've got a blank copy of the 1A that I use to use when re-enacting. I took it from the PIF and whited out the already filled in sections. (this was before photoshop!) Made a stack of them. If I can find it, I could send you a scan.

I haven't pulled them out in a few years, but I know it was one-sided and about 8.5 x 11.

I've drawn up the form 1A and I'm working on the form 1. I beleive the font is correct and the forms look like the original. If anyone wants a copy pm me and I can e-mail them to you.
