Well, Gryder is a little harsh to some but the VAC guys really screwed up. They give the appearance of not having a culture of excellence, professionalism, or safety. The FAA demands a culture of safety.

IF the pilot knew he had a significant leak, that accusation
might stick to him. I don't see how anyone else should be implicated.
